Annual Otis Oregon Pizza Eating Contests    ENTRY FORM, RULES & release of liability for eating contest 2019  A $5.00 entry/ Donation. 100%ENTRY FORM & RULES 2019  Annual Otis Oregon Eating Contests    ENTRY FORM, RULES & release of liability for eating contest 2019  A $5.00 entry/ Donation. 100% of the moneys go to benefit the Pick Of The Litter Newport Or. It's all about the animals! Contest is open to only the first 21 people who register!    
  1st Place wins $100.00 CASH  2nd Place wins $100.00 Gift Certificate at Otis Pizzeria.  3rd Place wins a $50.00 Gift Certificate at Otis Pizzeria  
  All contestants must be registered and checked in by 11:00 AM on day of contest Contestants must be 18 or older. The object is to eat as fast as posable the first one who finish first wins.  (Water will be provided) Contestants will wait for the whistle to start -  Any food in contestants mouth at the final whistle is counted - provided it is then swallowed. You must hold the food down or you will be disqualified.    Dunking and crunching the food is permitted. Separating the food is permitted. Contestants must not touch any food until the whistle begins the contest. Any contestant caught breaking any of the rules will be disqualified. This contest can be changed or cancel any anytime without notice.  
  WAIVER.  I knowledge  that eating large amounts of food in a short time potentially hazardous and is an uncomfortable activity. I should not enter and eat unless I am medically capable and properly trained. I realize that this is in good fun and possibly bad taste, but I agree to be a good sport. I agree to abide by the judges final decision of that all of the contest rules are obeyed by . I assume all risks associated with eating in this type of event including but not limited to indigestion, that stuffed feeling or death by choking,  Contestants sometime get a general dislike for that type of food after the contest. And all such risks being known and appreciated by me. Having read this release of liability and knowing these facts and in consideration of you accepting my entry, I for myself and anyone enlisted to act on my behalf, release of liability and release Otis Pizzeria, their representatives, sponsors, and their successors from all claims of liabilities of any kind arising out of my participation in this event.    Contestants will acknowledge there are risks associated with this event, which include, but are not limited to personal injury, risk of illness and possibly death, risk of loss or damage to personal property. Contestant voluntarily enters The Eating Contest and in doing so assumes all of these risks. Contestant upon entry of this contest and upon signing this form hereby agrees to waive and release Otis Pizzeria. and all associates, Landlord, volunteers, sponsors, vendors as well as any of their affiliates and successors from any liabilities of any kind arising from this event. Contestant has read all of the above rules and agrees to follow them. Contestant also hereby swears that to the best of their knowledge they are physically and mentally fit to compete in this contest.
